Procedures

1 . Sign in the log book

AGENCY ACTION:  Give the log book 

FEES: None 

PROCESS ING TIME:  1 minute

Municipal Civil Registrar/ Admin Asst I

(MCRO) 

2 . Proceeds to MCRO, approaches the frontline staff and submits the duly verified petition, copy of birth/marriage/ death certificate and the other supporting documents

AGENCY ACTION:  Receives, reviews the petition for completeness of entries and the authenticity of the supporting documents Instructs the client to pay the corresponding fees  

FEES: None 

PROCESS ING TIME:  15 minutes

(MCRO) 

3 . Pays for the corresponding fee/s to the Municipal Treasurer’s Office

AGENCY ACTION: 

FEES:  Petition Fee – P3,000.00 (CFN) 

PROCESS ING TIME:  5 minutes

Revenue Collection Clerk

(MTO) 

4 . Submits the official receipt to the MCR

AGENCY ACTION:  Issues Notice of Publication

Acts on the petition within 5days after the 10- day posting period Endorses the general approved or denied petition to the Civil Registrar General Informs the petitioner of the status of his/her petition upon receipt of the Civil Registrar General’s(CRG)

decision
