Availment of LGU Scholarship Program (Grade 7-10)

 1.    Form 137-A (1 photocopy) -secure this document from your school
2.    Photocopy of School ID -secure this document from your school
3.    Photocopy of Vaccination Card -secure this card from the Municipal Health Office
4.    Certificate of Indigency (1 copy original) -secure this document from your barangay

This scholarship program is intended for the indigent but deserving students who are residents of Baroy, Lanao del Norte.

Who may avail: 
Grade 7-10 indigent but deserving students with a general average of 82% 

Sign the logbook and return it after to the Administrative Aide I (Mayor’s Office) 

Submit required documents to Administrative Aide I (Mayor’s Office) 

Fill-out the assessment form

Administrative Assistant I (Mayor’s Office) shall:

3.1 Provide assessment form for evaluation

3.2 Interview the client

If lacking documents, return to the client 

Leave contact and wait for the schedule of release of school supplies (If qualified, enlist the student in the said program. )
